Office Policies

Co-payments are due at the time of your service. Cancellations must be made at least 24 hours prior to the date and time of your appointment or a $50 charge will be assessed to your account.

Insurance Information

Our clinic accepts most health plans in this area as well as Medicare. Please bring your ID and insurance card with you to all appointments. We will submit the claims to your insurance company. You will then be billed any amount not covered by your insurance company. If you have any questions or concerns regarding a bill or possible error in billing, please call the office at 903-957-0275.
